COMPLETING ON BEHALF OF A CHILD/TEEN?
If you are completing this on behalf of a child or teenager who is being photographed then:
1. The child’s or teen’s information goes in the top “Talent Info” section
2. Your information (as the parent/guardian) goes in the following section
3. The signature field *must* be signed by the child/teen's parent/legal guardian - this is likely you (a minor cannot sign a legal document)
